SIMPLE PRICING STRUCTURE
Members are billed on a quarterly basis for Fam Membership or immediately for a-la-carte services. There is a one-time non-refundable Membership registration fee of $99.
There is no contract so you can cancel at any time after the first three months if you feel the practice is not the right fit for you. There are packages available to “tryout” the Membership – inquire about that with the Concierge.
There are never any co-pays for visits however for certain procedures/imaging/ immunizations/lab draws and specialists there may be a cost but this will passed through to you at greatly reduced rates whenever possible. You will be notified ahead of time if you will be responsible for any additional costs.
Fam Member Pricing and Services ($3.25 per member per day ie $99/ month – annual contract required :$1200)

- Care Coordination for specialist, imaging, labs, medications, and hospital based services with negotiated pricing whenever possible
- Complimentary Telemental Therapy Consultation
- Email and Text Access to Board Certified Physicians
- Discounted A-la-carte Services
- Invitation to exclusive events for Fam Members
- Direct Primary Care for Adults and Children (Fam Membership – Individuals) : $3.25 per family member per day
- Minimum 3 month contract required
Women’s Health for Fam Members -GYN; Primary Care; Urgent Care Bundle (<$8 per member per day ie $250/ month annual contract required: $3000 )
Includes:
- Dedicated access to team of Family Planning and Board Certified Gynecology Experts
- Medical oversight by Board Certified Family Medicine and Internal Medicine specials
- Back-up / Second option Ob Physician Service (Does not include delivery)
- Polycystic Ovary Syndrome (PCOS) center
- Unlimited access to visits, telemedicine, text, and email with team
- Coordination of specialist, imaging, and hospital visits along with negotiated discounted pricing when possible
- Fees: Fam Member Initiation fees + $7.50 per member per day (Inquire about Maternity package)
- Minimum 3 month contract required on Gyn membership
- **Obstetrics Memberships Available – 10 months – pre and post pregnancy. Dedicated Gyn for delivery. Contact for Pricing***

- Visit fee can be applied towards Gyn/ Primary Care membership after first consultation
- Obstetrics- 10 month membership available but not included in this package.
